Medication

If your child requires prescribed medication to be administered during school hours, please review the Administration of Medication Information prior to completing the Consent to Administer Medication Form.

After carefully reading the information, if applicable, please complete and return the following documentation to Enquiries@mhoc.catholic.edu.au

  • A completed Consent to Administer Medication Form.
  • Any other written medical authorisation signed by the prescribing health practitioner.

Medication needs to be dropped off before Term 1 begins, and medication will be required to:

  • Be in its original container with intact packaging.
  • Include a current pharmacy label indicating it is prescribed medication and/or written authorisation signed by the prescribing health practitioner.
